Insurance And Payment Information

By state law a physican referral, or prescription, is required to receive physical therapy care. Most, but not all, health care coverage has provisions for physical therapy. Our office staff can confirm whether or not physical therapy is covered by your provider. This is very important because ultimately the patient is responsible for the payment of services rendered. Rejection of claims for payment by an insurance company does not relieve a patient of any obligations to pay our clinic or our staff.

Please call our office to confirm that we accept your insurance. Our office staff will make every reasonable effort to work with you, and your providers, to provide excellent physical therapy services and minimize your out of pocket expenses.
